Download Waiver and Release Form
Download Medical Treatment Authorization Form (Required only for players traveling without Parents)

Note - To complete the registration please sign and mail forms along with the payment to 2012 Avalon Mist Circle, Dardenne Prairie, MO 63368. Payments will be payable to "American Cricket Academy", a 501c(3) nonprofit organization run by volunteers.
